Behavioral Health Team

 
Behavioral Health Coordinator (BHC)

Works side-by-side with primary care providers to integrate behavioral health into the medical visit by assessing mental health needs and coordinating psychiatry and therapy referrals. Can provide walk-in visits for acute needs or a crisis.


Patient Navigator

Works to minimize patient barriers to routine healthcare by helping them access River Valley Health & Dental resources. Coordinating referrals for community services, finding options for basic living needs, and collaborating with North Penn Legal Services to assist patients with legal needs.


Community Navigator

A Community Navigator is a member of the Behavioral Health and Social Services team who assesses barriers that may interfere with or prevent patients from receiving quality care. These barriers may include insurance or lack thereof, difficulty paying rent and utility, transportation, childcare, and more. The community navigator will provide patients with community resources to assist. The Community Navigator acts as a liaison with several agencies in the community.

Tele-Psych For Behavioral Health

 
River Valley Health & Dental is pleased to offer access to child, adolescent, and adult telepsychiatry medication management services for individuals who are actively enrolled in primary care at our Center. This specialty service is paid for by your Medical Assistance plan and can be accessed by talking with your primary care provider about your current mental health needs.

Tele-psychiatry offers diagnostic evaluations and medication management to our patients and collaborates with primary care providers to offer whole-person care.

“The goal of our telemedicine program for psychiatry is simple: Provide our patients access to specialty psychiatric care while also affording them
the privilege of shared access to their physical and mental health records by medical and psychiatric providers,” explains Sue King, licensed clinical social worker at River Valley Health & Dental.

River Valley Health & Dental contracted with InSight + Regroup, a telepsychiatry technology and services vendor, to provide psychiatric specialty care. River Valley Health & Dental has a registered Medical Assistant designated to work with the psychiatry providers to complete rooming, scheduling, securing consents and releases of information, and to be a conduit for communication between the psychiatrist and the patient’s medical provider in the office.
